Bhutan’s healthcare system has embarked on a remarkable transformation journey, leveraging the power of Information and Communication Technology (ICT) to enhance healthcare access, quality, and efficiency. This article explores the convergence of Bhutan’s healthcare landscape with the digital age, encompassing infrastructure, policy, and the ambitious Electronic Patient Information System (ePIS) project.Â
In 2019, Bhutan’s healthcare landscape served a population of approximately 727,143 people across 20 districts. It boasted 46 hospitals, including three regional referral hospitals and a national referral hospital. Complementing this infrastructure were Primary Health Care Centers, a dedicated workforce of over 5,000 health professionals, and a reliance on more than 20 IT systems to manage healthcare services. The healthcare system conducted 24,687 surgeries, 1,016,477 laboratory tests, and provided 219,554 radiology department services during the year.
Bhutan’s commitment to healthcare accessibility is deeply embedded in its constitution, particularly in Sections 21 and 22 under Article 9. These sections ensure free access to basic public health services, including traditional medicines, and guarantee security in cases of sickness, disability, or lack of adequate livelihood. Aligning with global health priorities, the World Health Assembly (WHA) underscored the importance of digital technologies in healthcare, emphasizing their role in advancing Universal Health Coverage and the Sustainable Development Goals.
To harness the potential of ICT in healthcare, Bhutan established the ICT Division within the Ministry of Health (MoH) in 2016. In 2017, the Emergency Medical Service Division of MoH’s Department of Medical Service handed over the ePIS Project to the ICT Division. The eHealth Strategy, developed in 2018 in collaboration with the Asian Development Bank (ADB) and the World Health Organization (WHO), laid the foundation for Bhutan’s digital healthcare transformation. Key existing e-health solutions include DHIS2, Disease Surveillance System, Laboratory Information System (LIS), Picture Archiving and Communication System (PACS), Health Help Center (HHC) system, Telemedicine solutions, Blood Transfusion System, DocApp (Mobile App), and Inpatient Department (IPD) Reporting System. Despite these advances, challenges remained, including paper-based approaches, limited patient data platforms, and suboptimal treatment outcomes. In response, Bhutan initiated the ePIS project to provide improved data access, generate evidence-based information for healthcare decisions, and contribute to the goal of Universal Health Coverage (UHC).
The ePIS project is a pivotal component of Bhutan’s 12 Five Year Plan’s Digital Drukyul Flagship Project, jointly implemented by the Ministry of Information and Communications (MoIC) and MoH, supported primarily by the Asian Development Bank, with co-funding from the Royal Government of Bhutan. The project emerged following the Royal Bhutan Government’s cabinet directive in January 2020, leading to its award to Druk Holdings and Investments (DHI) through a direct tender process, subsequently outsourced to Thimphu Tech Park Limited (TTPL) following Bhutan’s Procurement Rules and Regulations for sustainability.Â
Bhutan’s digital healthcare revolution, driven by the ePIS project and guided by constitutional principles, reflects a commitment to providing accessible, high-quality healthcare services to its citizens, while preserving its traditional healing heritage. This transformative journey promises a brighter, more equitable future for healthcare in Bhutan.

ePIS Project Pilot
The ePIS Project was piloted at Paro hospital with support from WHO by Emergency Medical Service Division of Department of Medical Service under Ministry of Health.Â

ePIS Project Formulized
With formation of ICT Division, project was entrusted with the ICT Division to be implemented as flagship project with support from ADB and co financing from RGOB. Also supported by WHO.

Design Thinking for ePIS & eHealth Strategy
Many attempts to implement system (HIS & Telemedicine) in hospital was not successful in the past. Assessment using Design Thinking methods with support from RCSC was carried out. The findings from the assessment was used to strategize the impanation.Â
At the same time National eHealth Strategy was developed with support from ADB and WHO.

Cabinet Directive
The MoH received directive from Cabinet Secretariate to award the system development and implantation work to DHI. MoH caried out the directive and awarded the work to DHI who in turn had their subsidiary company TTPL to carry out the work

MoH request for RFP from TTPL
MoH received expressing of interest (EoI) from TTPL after which MoH share the ToR and requested for RFP from TTPL.

ePIS Project Contract Signed
The TWG reviewed the proposal and then the contract was signed between MoH and TTPL in witness of Hon’ble Health Minister and TTPL Chairperson on 21st April 2021.Â

Digital Literacy for Health Workforce and ePIS Assessment
The assessment result highlighted IT skill gap among many health professional. In order to prepare them for upcoming epis system, digital skilling was carried out with support from GovTech. Â

Project Charter Developed
A Project charter was developed where roles of MoH and TTPL was defined and agreed upon. The charter was endorsed by Project PSC.

ePIS SRS Finalization
The project PMU and TTPL conducted endless meeting with end user starting from MoH officials including all hospital staff to PHC and Sub Post staffs to collect requirement for the system. The system prototype was demonstrated to user and System Requirement Specification was signed in August 2021.

ePIS Trial at JDWNRH
The first few module consisting of registration, doctors desk only and piloted in the evening OPD of JDWNRH.

Civil Service Reform
The Parliament passed the Civil Servant Reform where one of the results included dissolvement of the Ministry of Information and Communications which oversaw the IT initiatives under Department of IT & Telecom. A new agency call Government Technology was established where both IT personnel and all activities are centralized . Â

Health Sector Reform
The Health sector reform was initiated where NMS was formed and ICT Division of MoH was dissolved and all IT professional along with ePIS Project sent to GovTech. A new Project Steering Committee was formed where NMS President was the Chairperson of the Committee. The Project TWG was also dissolved and the Heads of JDWNRH was appointed as the new TWG for the project.

ePIS Phase I Go live at JDWNRH
The Health Minister launched the system on April 14, 2023. The Phase I service included Administration, Appointment, Reception, Doctor Desk, Radiology, Laboratory, Pharmacy, Traditional Medicine Billing
and MIS Reports.

Rollout Phase II
In Nov., 2023 Priority IIÂ of Phase I was implemented at JDWNRH. The service included Inpatient Department (IPD), Operation Theatre, Community Health, Physiotherapy, Dialysis and Forensic.

Rollout in 6 Health Facilities
By Dec., 2023 ePIS was rolled out in 6 hospitals which included JDWNRH and National TM Hospital at Thimphu; Â Eastern Regional Referral Hospital and Gyalpoizhing Hospital in Mongar; Paro Hospital and Central Regional Referral Hospital at Gelephu.
